This should have been part of
https://github.com/inducer/loopy/pull/884 / 87d6ccfd
but was missed.

Without this, data from dictionary types that are not installed can
linger in caches and fail to unpickle when unpickling is attempted.

* replace pyrsistent.PMap, immutables.Map with immutabledict

* go back to immutables.Map for Tree

* work around doc failure

* remove some spurious changes
