Torus refinement does not finish succesfully
The issue is that it doesn't manage to actually finish refininig, i.e. after the default 10 iterations it still has disturbed expansions and they're all clustered along the z = 0 line. It seems to succeed if the expansion_disturbance_tolerance is bumped to something like 0.1, which seems unreasonable.
This script reproduces the problem for me on current master commit 1c153f3d:
