Make sure sufficient quadrature resolution is supplied at all QBX orders

@howard28 tried QBX order 7 in !87, and, with the new quad resolution estimates from !95 (merged) (see !121 (closed) for context), encountered a situation where not enough quad resolution was provided. For increased black-boxiness, the quadrature resolution (fine_quad_order) (if not supplied) should adjust itself to suit the QBX order.
